Overview
This role involves working in partnership with a physician to assess and manage various acute and chronic health issues within designated populations.
Key Responsibilities
  • Gather patient medical history and conduct assessments through observation, interviews, and physical examinations.
  • Order, execute, and analyze diagnostic tests.
  • May conduct preventive health evaluations, screenings, immunizations, and other care services.
  • Deliver direct treatment and management of health issues through prescriptive authority or referrals to other healthcare professionals or community resources.
  • Educate and counsel patients and their families/caregivers on preventive health, treatment alternatives, and available community resources.
  • Provide care that respects age and cultural considerations, promoting informed choices, shared decision-making, and self-determination.
  • Ensure accurate and timely documentation of assessments and patient care plans.
  • May assist with registration tasks, including collecting and entering demographic, insurance, and payment information.
Qualifications
  • Master's degree or higher in Nursing is required upon hire.
  • No prior experience is necessary.
  • Must hold a valid RN license to practice nursing in the State of Kentucky upon hire.
  • Must possess an APRN license as an Advanced Practice Registered Nurse in Kentucky upon hire.
  • National certification from one of the following is required upon hire: American Academy of Nurse Practitioners (AANP), American Association of Critical Care Nurses (AACCN), National Certification for the Obstetric, Gynecologic, and Neonatal Nursing Specialties (NCCCOGN), Pediatric Nursing Certification Board (PNCB), American Nurses Credentialing Center (ANCC), or as a Neonatal Nurse Practitioner from the National Certification Corporation (NCC).
Skills and Attributes
  • Strong critical thinking abilities and sound judgment are essential.
  • Ability to work independently with minimal supervision.
  • Capable of functioning effectively in a high-pressure environment and taking appropriate actions.
Physical Requirements
  • Frequent standing and walking.
  • Occasional sitting.
  • Rarely lifting or carrying weights over 25 lbs.
  • Frequent pushing/pulling of weights up to 25 lbs.
  • Occasional climbing, bending, kneeling, and crouching.
  • Frequent reaching, talking, and hearing.
  • Regular use of repetitive hand/arm movements and keyboard data entry.
  • Frequent depth perception and color distinction abilities required.
