Essential Functions & Responsibilities:

  • Responsible for managing the clinical care of patients at assigned skilled nursing facilities (SNFs).
  • Conduct daily rounds and prescribe necessary medications and treatments.
  • Deliver high-quality, patient-centered primary care services.
  • Ensure timely documentation of all patient interactions.
  • Available for consultations with facility nursing staff via phone or secure messaging.
  • Utilize Theoria's proprietary electronic health record (EHR) system, ChartEasy, for documentation.
  • Familiarity with commonly used facility EHR systems such as PointClickCare, MatrixCare, and American HealthTech.
  • Expected to be punctual and present for scheduled shifts.
  • Reliable transportation is necessary.

Shift Structure:

  • Rounding occurs from 7 AM to 1 PM local time, during which the provider must be accessible to nursing staff and address any urgent issues.
  • Shifts typically commence around 7 AM local time, with reasonable flexibility.
  • Shifts are scheduled Monday through Friday.
  • Charting and documentation can be performed off-site.
  • This is an in-person role.
