You will play a crucial role in assessing, coordinating, planning, directing, implementing, and managing the needs of assigned patients throughout their episode of care. Your focus will be on progress toward discharge, including during transitions within the acute care stay.

You will also be responsible for the delivery and coordination of patient care provided by the caregiving team. Your commitment to ensuring that competent, compassionate patient care is uniformly provided to customers, including referring facilities, hospital staff, and patients in various settings, is essential.

We offer a competitive hourly rate between $33.00 and $33.99, providing a stable financial foundation for your career growth.
