Salary and Benefits

We offer a competitive salary of $85,000 - $110,000 per year, depending on experience, plus a comprehensive benefits package, including medical, dental, and vision coverage, 401(k) matching, and paid time off.

Job Description

As a Care Coordinator RN, you will be responsible for assessing patient and family strengths and needs, developing individualized plans of care, and managing transitions between settings. You will also collaborate with healthcare teams to ensure effective communication and coordination of care.

  • Assesses existing or incoming referral patient/family strengths and needs.
  • Develops a family-centered plan of care as a member of the interdisciplinary team.
  • Manages transitions between settings, caregivers, and providers.
Required Skills and Qualifications

To be successful in this role, you must have:

  • A BSN degree from an accredited nursing program.
  • Current licensure as a registered nurse in the state of Ohio.
  • At least two years of nursing experience, preferably in pediatric nursing.
