Job Summary

We are seeking a highly skilled and compassionate Care Coordinator to join our team at WestCare Foundation. As a key member of our staff, you will play a vital role in providing direct services to children exposed to violence and their non-abusing parents.

Key Responsibilities
  1. Provide individualized support and advocacy to clients, ensuring their needs are met and their rights are protected.
  2. Coordinate services with external agencies, including linkages and referrals, to ensure a seamless continuum of care.
  3. Conduct thorough needs assessments to identify client strengths and areas for improvement.
  4. Develop and implement personalized service plans, working collaboratively with clients and other stakeholders.
  5. Collaborate with treatment team members to develop discharge plans, ensuring a smooth transition for clients.
  6. Provide education and support to clients on case management and service planning.
  7. Maintain accurate and confidential client records, adhering to regulatory standards.
  8. Participate in project-related training, interdisciplinary team meetings, and community task forces to stay up-to-date on best practices and industry developments.
Requirements

To be successful in this role, you will need:

  • Certifications/Licensures: CAP, CPRS, or CPMH eligibility (or ability to obtain within 2 years of employment); CPR/First Aid certification within 6 months of employment and every two years thereafter.
  • Education: Graduation from an accredited four-year college with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology, or related field.
  • Experience and Competencies: Two years of related work experience in Mental Health and/or Substance Abuse field; ability to meet requirements of Monroe County Sheriff's Office background check; CPR Certification, First Aid Certification, and annual Tuberculosis test; ability to provide an Affidavit of Good Moral Character; clearance in Pre-Employment Drug Testing and Criminal Records Checks, as required.
