- Conduct thorough music therapy assessments to identify patient needs and develop personalized interventions.
- Collaborate with the interdisciplinary team to prioritize referrals and ensure effective care coordination.
- Maintain accurate and detailed documentation of all care provided, adhering to Ocean Healthcare's documentation requirements.
- Develop and implement education programs to enhance hospice staff understanding of music therapy's benefits and applications.
- Support the facilitation of support groups as needed, ensuring optimal resource utilization.
- Valid New Jersey MT license.
- Prior experience in hospice care or a related field is highly desirable.
- Reliable transportation is essential for this role.
