Key Responsibilities

The Nurse Practitioner (NP), operating under the supervision of the Medical Director and Practice Administrator, will deliver healthcare services in a family practice setting, adhering to their professional scope of practice.

The ideal candidate will be able to:

  • Conduct thorough health assessments through patient and family interviews, chart reviews, and comprehensive physical examinations.
  • Formulate and execute treatment plans, including prescribing medications in accordance with medical guidelines and state regulations.
  • Diagnose various health conditions and provide suitable treatments.
  • Work within defined parameters while consulting with the Medical Director as necessary.
  • Order and perform relevant laboratory tests and screenings, collaborating with other healthcare professionals for comprehensive patient evaluations.
  • Document assessments and treatment plans accurately and promptly in patient records.
  • Maintain and update medical records, including examination results, diagnoses, medications, and treatment plans.
  • Engage effectively with patients and families to communicate treatment plans and promote health management and disease prevention.
  • Identify and recommend community resources to support patient and family needs.
  • Collaborate with care teams to prepare for patient visits and ensure follow-up care.
  • Foster a cooperative and respectful environment with clinic staff, providers, and patients to enhance continuity of care.
  • Share pertinent information with other healthcare professionals and community organizations.
  • Facilitate appropriate discharge planning in collaboration with the interdisciplinary team to ensure ongoing care post-discharge.
  • Utilize telehealth services as needed to support patient care.
  • Perform additional duties as assigned.

Qualifications

  • Master's Degree or Doctorate in Nursing is required.
  • A minimum of 2 years of experience as a Nurse Practitioner in a pediatric or clinical setting is required.
  • Valid Registered Nurse and Nurse Practitioner licenses in Louisiana are required.
  • Must maintain all necessary approvals to prescribe and dispense medications under applicable Louisiana State and federal law.
  • Experience with electronic medical records (EMR) is essential.
  • Excellent written and verbal communication skills are necessary.
  • A courteous and compassionate bedside manner is expected.
