The Opportunity

We're seeking a part-time or full-time Nurse Practitioner to join our team in the Bay Area. As a key member of our primary care team, you'll provide onsite services to older adults, develop person-centered care plans, and work closely with our Medical Director to address emerging issues and develop policies that support our clinical guidelines.

Key Responsibilities:
  • Provide primary care services to seniors in Senior Living Communities
  • Develop and execute detailed person-centered care plans
  • Collaborate with Medical Director to identify and address emerging issues
  • Work with Primary Care Coordinators to provide Chronic Care Management visits and review documentation
  • Support diagnostic needs with Traveling Clinical Assistants
  • Lead regular huddles and provide clinical leadership
  • Identify opportunities to work with senior living management and care teams to improve patient outcomes
Requirements:
  • Board-Eligible or Board-Certified Physicians in Internal Medicine or Family Medicine, or Nurse Practitioner or Physician Assistant with geriatrics experience
  • Comfortable with a fast-paced, changing environment
  • Ability to provide onsite clinical care across the Bay Area
  • Strong orientation towards collaboration and working with a multidisciplinary team
  • Strong written and verbal communication skills
  • Ability to work with tech-enabled care delivery tools

The salary range for full-time for this role is $140,000 to $180,000 inclusive of base, annual performance bonus, sign-on bonus, plus equity and benefits.
