Overview
We are seeking a skilled Physical Therapist to join our Outpatient at Home Program in Bardstown, KY. As a member of our team, you will provide high-quality physical therapy services to patients in their homes, working closely with caregivers and facilities to ensure seamless care.
Responsibilities
- Develop and implement individualized treatment plans for patients, utilizing evidence-based practices and collaborating with healthcare teams to ensure comprehensive care.
- Conduct thorough assessments, establish goals, and provide ongoing therapy sessions to patients, promoting optimal recovery and functional independence.
- Maintain accurate records, communicate effectively with patients, caregivers, and healthcare professionals, and contribute to a positive, supportive work environment.
Qualifications
- Graduate of an American Physical Therapy Association (APTA) accredited school of physical therapy.
- At least one year of experience in outpatient physical therapy, with a focus on orthopedic rehabilitation.
- Valid State Physical Therapy License/Registration required.
