As a key member of our clinical team, you will provide high-quality patient care across the entire continuum of care, making a meaningful impact in the lives of our patients and their families.

Key Responsibilities
  • Deliver patient-centered care, prioritizing the needs and preferences of each individual
  • Develop and implement personalized care plans, collaborating with interdisciplinary teams to ensure seamless care transitions
  • Conduct thorough assessments, identifying and addressing physical, emotional, and spiritual needs
  • Provide education and support to patients and their families, empowering them to make informed decisions about their care
  • Collaborate with healthcare providers to ensure timely and effective care coordination
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements
Requirements
  • Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
  • Current State Licensee as a Registered Nurse RN
  • 1 year of experience in a clinical care setting, with a preference for hospice or home health experience
  • Nursing skills as defined by accepted nursing standards
  • Ability to handle multiple priorities, documentation requirements, and deadlines
  • Reliable transportation to perform job tasks
