Key Responsibilities- Engagement and Exploration: Engage patients and their families in a helping process, assessing their psychosocial and mental health needs to identify barriers to treatment and enhance their health status.
- Goal Development and Implementation: Develop a comprehensive plan of intervention in collaboration with patients, families, and the healthcare team, implementing a plan of care and monitoring patient response.
- Evaluation and Termination: Briefly summarize care plans with patients and families, provide linkages to community resources, and participate in process improvements to evaluate patient outcomes.
- Education and Consultation: Provide consultation and training to medical staff and other healthcare professionals on psychosocial issues, collaborating with coworkers and management to ensure the ongoing development of the Social Work Department.
- Education: Master of Social Work from an accredited School of Social Work.
- License and Certification: Licensed by the State of Ohio Social Work Board (LSW, LISW, or LISW-S).
- Years of Experience: One to two years of equivalent experience, with three to five years preferred.
- Required Skills and Knowledge: Casework principles, including diagnosis, assessment, crisis intervention, and treatment planning, as well as techniques in individual, family, and group therapy.
