**Key Responsibilities:**
  • Provide direct patient care to residents in a Long Term Care/Skilled Nursing Facility setting, including administering medications, performing treatments, and providing wound care as needed.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident.
  • Monitor vital signs, assess patient condition, and document care provided to ensure accurate and timely communication with the healthcare team.
**Requirements:**
  • Current RN license in good standing.
  • Experience in a Long Term Care/Skilled Nursing Facility setting, with a strong understanding of geriatric care and rehabilitation principles.
  • Ability to work in a fast-paced environment and prioritize tasks effectively to meet the needs of our residents.
  • Strong communication and interpersonal skills, with the ability to work collaboratively with the healthcare team to achieve optimal patient outcomes.
**Work Environment:**
  • 12-hour shifts, 6a-6p/6p-6a.
  • Every other weekend and rotated holidays.
  • Block scheduling may be considered, dependent on facility needs.
