Position Summary:
We are in search of a skilled and driven individual to join our team as a Clinical Pharmacist. This role offers the chance to enhance your pharmaceutical expertise under the mentorship of a licensed pharmacist while providing critical care to our patients in a closed-door pharmacy environment.

Work Schedule:
Monday - Friday
8am - 5pm / 11am - 7pm
Rotating weekends
8am - 4pm

Key Responsibilities:
- Assess physician medication orders for appropriateness regarding drug selection, dosage, potential interactions, and administration routes.
- Recommend therapy modifications to physicians and facility staff as necessary.
- Maintain an accurate patient/resident database, including current and historical medical information, clinical data, and concurrent medications.
- Ensure that necessary laboratory tests for monitoring high-risk conditions are ordered and evaluated in collaboration with the healthcare team.
- Provide clinical and technical support to the healthcare team as required.
- Document all communications with healthcare professionals and patients in the medical record.
- Implement policies for the safe handling, storage, distribution, and disposal of medications.
- Maintain compliance with federal and state regulations and organizational policies.
- Report medication-related errors to the Pharmacy Manager.
- Assist facility staff with procedures related to the safe procurement and administration of medications.
- Perform additional tasks as assigned.

Qualifications:
- Bachelor's Degree in Pharmacy; Pharm D preferred.
- Minimum of one year of experience in long-term care.

Licenses/Certifications:
- As required by regulatory standards and business needs.
- Detail-oriented, goal-driven, and a collaborative team player with strong oral communication skills.
