Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Visiting Nurse. As a Visiting Nurse, you will play a vital role in providing patient-centered care to individuals in need of home healthcare services.

Key Responsibilities
  • Provide high-quality patient care and support to individuals in their homes, including wound care, medication management, and education on disease management.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop personalized care plans to meet those needs.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Monitor patients' progress, adjust care plans as needed, and provide ongoing education and support to patients and families.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.

Requirements
  • Valid CT RN license.
  • One year of RN experience, with a preference for homecare experience.
  • Valid driver's license and reliable transportation.
  • Computer proficiency and ability to navigate electronic medical records.
