Join Our Team as a Nurse Practitioner in Long-Term Care

We are seeking a skilled and compassionate Nurse Practitioner to join our team at Altercare Integrated Health Services. As a key member of our Institutional Interdisciplinary Care Team, you will provide high-quality care to our residents in long-term care facilities.

Responsibilities:
  • Triage care needs based on the Health Risk Assessment Tool (HRAT) scores and conduct ongoing clinical assessments.
  • Provide input into the development of the Individualized Care Plan (ICP) in collaboration with the case manager and other members of the ICT.
  • Interface with other practitioners to facilitate access to all services, including IP, SNF, specialist care, and behavioral health.
  • Visit patients as scheduled and after hospital discharge to evaluate progress, conduct physical exams, prescribe interventions, and communicate results to staff.
  • Evaluate episodic complaints and initiate treatment as needed.
  • Obtain consultation and diagnostic reports and review as needed with the Medical Director and ICT.
  • Identify and address changes in the members' health status by being available on call when not at the NF during scheduled visits.
