In this role, you will conduct in-home assessments, medication reviews, and health screenings for our members. You will also provide patient education, identify diagnoses, and develop care plans in collaboration with our Regional Medical Officer and other team members.

**Responsibilities:**
* Conduct in-home assessments, medication reviews, and health screenings for members
* Provide patient education and develop care plans in collaboration with the Regional Medical Officer and other team members
* Identify diagnoses and develop care plans to prevent unnecessary hospitalizations and health complications
* Partner with multiple stakeholders, including PCPs, specialists, and ancillary providers
* Lead broader clinical teams, including nurses, health coaches, social workers, and care coordinators
**Requirements:**
* Master's degree from an accredited NP Program or PA program
* Preferred one year of prior clinical or home care experience
* Previous EMR experience preferred
* Active California state Nurse Practitioner or Physician Assistant license
* Active RN License and Furnishing number (Must, upon hire) NP only
* Active Nurse Practitioner Board Certification/Physician Assistant Certification
* NPI Number, DEA, Valid BLS
* Valid California driver license and current automobile insurance
* Experience in care of older adult (geriatric) patients preferred
* Excellent administrative, organizational, and communication skills required
