Key Responsibilities:

  • Conduct in-home assessments, including medical history reviews, medication reconciliation, and comprehensive physical exams
  • Perform evidence-based practice screenings, including point-of-care testing
  • Identify diagnoses and develop care plans to improve patient outcomes
  • Communicate findings with patients' primary care physicians to ensure seamless care
  • Educate patients on disease management, medication adherence, and healthy lifestyle choices

Requirements:

  • Active NP or PA license; ability to obtain by start date
  • Current national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care
  • Ability to complete physical demands of the job, including lifting and navigating various dwelling conditions
  • Reliable transportation for home visits

Preferred Qualifications:

  • Experience in gerontology, cardiology, internal medicine, or endocrinology
  • Home health care or home visit experience
  • Ability to travel up to 5% within licensed states for additional incentives

Benefits:

  • $40,000 Student Loan Repayment or $20,000 Sign-on Bonus for eligible candidates
  • Flexible provider schedules available between 20-40 hours per week
