Job OverviewWe're seeking a highly skilled Home Health Nurse Practitioner to join our team as a Community Care Specialist. As a key member of our interdisciplinary team, you'll play a vital role in delivering exceptional clinical outcomes to our patients in the comfort of their own homes.
Responsibilities- Monitor and follow patient progress, reporting health changes to supervising registered nurses through detailed observations, interviews, and physical/psychosocial care within LPN scope of practice.
- Demonstrate personalized nursing and care management by constantly evaluating evolving patient needs and adjusting the plan of care.
- Practice quality-driven and care coordination by communicating with the VNS Health interdisciplinary team and across providers and other resources to deliver the plan of care effectively and efficiently.
- Use technology, including tablet computers, phones, and electronic medical records (EMRs), to coordinate and maintain accurate and up-to-date patient information.
- Provide empowering education and support to patients and their families to help them understand and navigate end-of-life treatment options.
- Licenses and Certifications: License and current registration to practice as an LPN in New York State, required.
- Education: High School diploma and completion of LPN program, required.
- Work Experience: Minimum of one year of work experience as an LPN, required.
- Demonstrated ability to solve problems and think critically to support patient outcomes, required.
- Experience with electronic charting in HHAeXchange, preferred.
- A competitive salary range of $62,000 - $80,000 per year, depending on experience.
