Job Summary

We are seeking a skilled Home Health Aide II to provide high-quality care to our clients in their homes. As a member of our Home Health Team, you will be responsible for assisting with daily living activities, observing client condition, and documenting changes according to the Care Plan.

Key Responsibilities
  • Provide personal care services, including bathing, grooming, ambulating, and transferring, for varied time frames
  • Observe client condition and document changes according to the Care Plan
  • Work collaboratively with the Home Health Team to ensure seamless care delivery
  • Cover weekends and team member PTO as needed
  • Perform office tasks and travel outside normal work areas to meet patient caseload needs
Requirements
  • High school diploma or equivalent
  • Two years of experience as a Home Health Aide required
  • BLS (American Heart Association/Basic Life Support) certification required
  • HHA (Home Health Aide) certificate or class curriculum if CNA applies
  • Valid Florida driver's license within 30 days of employment
