AccentCare is seeking a compassionate and skilled Nurse Case Manager to join our Home Health team. As a RN Case Manager, you will play a vital role in delivering high-quality patient care and ensuring the best possible outcomes for our patients and their families.

Key Responsibilities:

  • Conduct thorough assessments of patients' medical and social needs
  • Develop and implement individualized care plans to meet patients' unique needs
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Provide education and support to patients and their families

Requirements:

  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation
  • One (1) year experience as a RN
  • Valid CPR certification
  • Licensed driver with an automobile that is insured in accordance with state and/or organization requirements
