Job Summary

We are seeking a skilled and compassionate LPN to provide one-on-one care to a medically fragile student in a school setting. As a Special Needs School Health LPN, you will play a vital role in ensuring the student's health and well-being, while also collaborating with educational professionals to promote healthy relationships between community health providers and students, parents, and the community at large.

Key Responsibilities
  • Assess children's health needs to obtain necessary information for medical services and develop health plans to meet their functional and health needs.
  • Provide one-on-one care to a medically fragile student in their classroom and during transportation to and from school.
  • Administer medical intervention services, first aid services, and medication in accordance with school district and PSI policies.
  • Complete and maintain required documentation of student health records.
  • Collaborate with educational professionals to promote healthy relationships between community health providers and students, parents, and the community at large.
Requirements
  • Current LPN license through the Ohio Board of Nursing.
  • Experience working with medically fragile children is strongly preferred.
  • Knowledge of communicable disease prevention and medication protocol.
  • Knowledge of HIPAA laws and ability to handle confidential information appropriately.
  • Experience with trach care, vent care, tube feeding, and catheterization is strongly preferred.
  • Collaborative, constructive team building skills are required.
  • Strong communication and interpersonal skills are required.
  • Excellent organizational skills and time management skills are required.
