Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our team as a Compassionate Care Specialist. In this role, you will provide professional nursing care to patients in their homes, utilizing the nursing process to assess, plan, implement, and evaluate patient care.

Key Responsibilities
  • Assess patient needs and develop individualized care plans to meet their unique needs and goals.
  • Provide high-quality patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Document patient care and services provided in accordance with Duke Health policies and regulatory standards.
  • Participate in ongoing education and training to maintain current knowledge and skills in patient care and home health services.
Requirements
  • Current North Carolina RN licensure or compact RN licensure in participating state.
  • Current State of North Carolina Cardio-Pulmonary Resuscitation Certificate (CPR) (American Heart Association only).
  • Two years of med-surgical, critical care, or home health/hospice/infusion experience.
  • Valid driver's license and current automobile insurance coverage.
Preferred Qualifications
  • Associate Degree or Diploma, Nursing required.
  • Registered Nurses are not required but encouraged to enroll in a DUHS approved BSN program after completing two years of service of their start date.
  • Prior experience in Home Health, Hospice, or Infusion preferred.
  • Prior OASIS and PDGM experience preferred for home health.
