About the Role

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in providing high-quality care to clients in their homes.

As a Physical Therapist, you will be responsible for making home visits to clients, performing diagnostic tests and measurements, and developing and implementing individualized care plans.

You will work closely with the multidisciplinary healthcare team to ensure that clients receive the best possible care and support.

Key responsibilities include:

  • Conducting home visits to assess clients' needs and develop care plans
  • Performing diagnostic tests and measurements to inform care decisions
  • Developing and implementing individualized care plans to meet clients' needs
  • Collaborating with the multidisciplinary healthcare team to ensure seamless care delivery
  • Providing education and support to clients and their families to promote independence and well-being

Requirements for the role include:

  • A current PA Physical Therapist license
  • A minimum of one year of recent work experience as a Physical Therapist
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
  • Ability to work independently and manage time effectively
  • Strong interpersonal, organizational, and problem-solving skills
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred
