At ScionHealth, we are seeking a skilled RN Case Manager- Per Diem Weekdays to join our team. As a key member of our healthcare team, you will be responsible for coordinating and facilitating the care of patients throughout their stay with us.

Job Summary

This role requires a dedicated and detail-oriented individual who can effectively communicate and collaborate with the Interdisciplinary Care Transitions (ICT) team members. You will work closely with patients, families, and healthcare providers to ensure seamless care coordination and efficient resource utilization.

Key Responsibilities
  • Coordinate clinical and psycho-social activities with the Interdisciplinary Team and Physicians.
  • Monitor patients' stays to ensure effective care coordination and efficient care facilitation.
  • Stay up-to-date on reimbursement modalities, community resources, case management, psychosocial, and legal issues affecting patients and providers.
  • Refer high-risk patients who would benefit from additional support.
  • Serve as a patient advocate and enhance collaboration to maximize patient and family decision-making abilities.
  • Demonstrate knowledge of growth and development principles and provide age-appropriate care to the patient population served.
Requirements

To be successful in this role, you will need:

  • Knowledge of government and non-government payor practices, regulations, standards, and reimbursement.
  • Familiarity with Medicare benefits and insurance processes and contracts.
  • Knowledge of accreditation standards and compliance requirements.
  • Critical thinking, prioritization, and time management skills.
  • Basic computer skills with working knowledge of Microsoft Office, word-processing, and spreadsheet software.
  • Excellent interpersonal, verbal, and written communication skills.
  • Good attendance and regular work habits.
