Independent Optometrist Job Description

As an Independent Optometrist at Sam's Club, you will have the autonomy to establish your own practice and provide high-quality eye care services to our members. This is a unique opportunity to build your own business and take control of your career.

Key Responsibilities:
  • Provide comprehensive eye exams and vision care services to members
  • Establish your own schedule and hours of operation
  • Set your own fee schedule and keep 100% of exam/professional fees
  • Have access to equipment and instrumentation for a fair market value
  • Option to opt into administrative and/or Opt-Tech support services (if state allows)
Requirements:
  • Doctor of Optometry (OD) from an accredited optometry school
  • Valid state optometry license
  • Ophthalmologist (MD or DO) from an accredited school (for certain locations)
  • Valid state ophthalmology license (for certain locations)
  • General Liability Certificate
  • Professional Liability Insurance Certificate
  • W-9
