As a Patient-Centered Healthcare Provider at ChenMed, you will be part of a dynamic team that values innovation, teamwork, and excellence in patient care.
Responsibilities:- Provide comprehensive patient care, including assessment, diagnosis, treatment, management, education, health promotion, and care coordination.
- Act as a leader in your care team, fostering a positive culture and promoting accountability for outcomes.
- Engage with patients, families, and other healthcare professionals to ensure seamless care transitions and optimal health outcomes.
- Stay up-to-date with evidence-based medicine and best practices in patient-centered care.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote excellent patient care.
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty preferred.
- Current State Medical License or eligibility to obtain a license in the state(s) where you will be working.
- Basic Life Support (BLS) certification required.
- Ability to travel locally, regionally, and nationally as needed.
The estimated annual salary for this position is $220,000-$280,000, depending on experience and location.
