Nurse Practitioner

Job Summary:
We are seeking a skilled and compassionate RN to join our Advanced Illness Management Transitions team. As a Home Health Nurse lll/Telehealth, you will provide high-quality, patient-centered care to individuals in their homes, utilizing a holistic approach that incorporates psychological, sociocultural, spiritual, economic, and lifestyle factors.
Responsibilities:
- Assess, coordinate, deliver, and evaluate in-home nursing care, including direct patient care, patient/family education, and transitions of care.
- Develop, implement, manage, and coordinate an optimal interdisciplinary plan of care that addresses the unique needs of each patient.
- Foster and maintain collaborative relationships with patients, families, physicians, and other healthcare providers through timely and effective communication.
- Adhere to policies, industry standards, best practices, and applicable laws and regulations to ensure a high-quality, reliable patient experience.
Requirements:
- Graduate of an accredited school of nursing.
- RN-Registered Nurse of California.
- BLS-Basic Life Support Healthcare Provider.
- California Drivers License.
- Valid Auto Insurance.
Preferred Experience:
5 years' experience working as an RN in a home health or ambulatory setting.
Skills and Knowledge:
- Demonstrated proficient knowledge and competence in nursing theories, concepts, and practices, medical terminology, anatomy, and physiology.
- Working knowledge of the healthcare industry, safety precaution policies, best practices regarding patient care and privacy, and changes in local/state/federal regulations.
- Excellent written and verbal communication skills to explain sensitive information clearly and professionally to diverse audiences.
- Proficient knowledge of computer applications, such as Microsoft Office Suite (Word and Outlook), Electronic Health Record.
- Time management and organizational skills, including the ability to prioritize patient care, assignments, and work within standardized policies, procedures, and practices to achieve objectives and meet deadlines.

$71.71 to $95.35 / hour

Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
