Responsibilities:
  • Deliver comprehensive nursing care to patients in accordance with their individualized plans of care.
  • Conduct thorough assessments and develop, implement, and evaluate plans of care in collaboration with the patient, physician, and care team.
  • Ensure quality and safe delivery of home health care services, adhering to all applicable laws, rules, and regulations.
  • Document all communications with patients, families, physicians, and other healthcare professionals.
  • Collaborate with the interdisciplinary team to develop and implement patient care plans.
  • Evaluate patient progress and outcomes, revising care plans as needed.
Requirements:
  • OASIS/Medicare Certified Visits experience.
  • Electronic charting experience with Finale Health.
  • Developed computer skills and problem-solving abilities.
  • Minimum of 1 year of recent home health nursing experience.
  • Valid U.S. driver's license and current auto insurance.
  • American Heart Association BLS certification.
  • Excellent communication and interpersonal skills.
