We are seeking a highly skilled MDS Coordinator to join our team at Liberty Village of Peoria. As a key member of our healthcare team, you will be responsible for ensuring the accuracy and completeness of resident data, facilitating care planning, and promoting quality care.
Key Responsibilities- Schedule and conduct MDS assessments, ensuring compliance with RAI manual guidelines.
- Complete assigned observations to support OBRA MDS assessments, including pain assessments and Braden scales.
- Audit and review ICD 10 coding on admission, selecting primary and admission diagnoses based on medical condition.
- Complete clinical sequencing for Medicare A and managed care residents.
- Manage MDS transmission process, ensuring accuracy and completeness of data.
- Participate in Medicare meetings, facilitating comprehensive care planning and quality improvement initiatives.
- Monitor and review Case Mix Index, Quality Measures, and 5-star reports, ensuring compliance with state-specific RUG category requirements.
- Occasional lifting and movement of up to 50 lbs.
- Specific vision abilities, including close vision, depth perception, and ability to adjust focus.
