Position Details

LOCATION: Lancaster, PA

HOURS: Full-time 0.8 FTE (64 hours/2 weeks); Monday- Friday; varied hours 7:30am-5:00pm; no holidays or weekends.

The Practice Nurse may rotate between our facilities to ensure adequate coverage during staff absences.

KEY RESPONSIBILITIES: Qualified candidates must be capable of performing the following tasks (with or without reasonable accommodation):
  • Deliver clinical guidance via phone and electronic communication based on clinical assessments and triage protocols, adhering to regulatory standards.
  • Provide tailored patient education on preventive care, chronic disease management, acute diagnoses, medications, and other relevant health topics.
  • Prepare patients for appointments using established rooming protocols.
  • Administer prescribed injections and medications in accordance with departmental guidelines and standards.
  • Assist providers with various procedures, minor surgeries, and examinations as required.
  • Conduct Point of Care Testing, including but not limited to hearing, flu, urine, and strep tests.
  • Collect blood and other lab specimens for further analysis.
  • Authorize prescription refills as directed by the provider.
  • Support test tracking and monitor overdue results.
  • Remove sutures and change dressings as necessary.
  • Ensure exam rooms are well-stocked and sanitized after each patient visit. Maintain and clean medical instruments and equipment post-procedure.
  • Collaborate with providers and patients to review and adjust care plans, assessing patient progress and following up as needed.
  • Engage in proactive outreach for patients requiring preventive and chronic disease services.
  • Facilitate care coordination among specialists and transitions of care to gather all necessary patient information.
  • Assist in coordinating referrals, education, and community resources.
  • Actively participate in initiatives aimed at enhancing patient experience and quality of care.
  • Participate in daily team meetings with the provider.
  • Serve as an advocate for patient experience, maintaining professionalism in all interactions with patients, visitors, and colleagues.
  • Handle all patient information confidentially in accordance with privacy policies.
ADDITIONAL RESPONSIBILITIES: The following tasks are considered secondary to the primary responsibilities:
  • Perform various administrative duties as needed.
  • May require some regional travel; a valid driver's license is necessary.
  • Participate in on-call duties as required.
  • Other duties as assigned.
MINIMUM QUALIFICATIONS:
  • Completion of a LPN or RN educational program.
  • Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) issued by the PA State Board of Nursing.
  • AHA CPR/BLS certification required within 30 days of hire.
  • This role requires occasional regional travel. Ability to travel to multiple sites is essential.
PREFERRED QUALIFICATIONS:
  • One (1) to Two (2) years of nursing experience in a primary or specialty care setting.
  • Prior experience in triage, emergency care, cardiac care, intensive care, or related nursing specialties.
