Job Summary

Oak Medical is seeking a dedicated and compassionate Senior Living Internal Medicine Nurse Practitioner (NP) or Physician Assistant (PA) to join our professional team in the Waukesha area. As a key member of our team, you will deliver high-quality patient care in Skilled Nursing, Memory Care, Assisted Living, and Independent Living environments.

Key Responsibilities
  • General Medical Care: Provide comprehensive medical care to adult and geriatric patients, addressing both acute and chronic primary care complaints.
  • Evaluations: Conduct thorough evaluations, including history taking and physical examinations, to establish and document accurate medical diagnoses.
  • Diagnostic Testing: Order and interpret relevant diagnostic laboratory and radiology tests to support medical diagnoses and treatment plans.
  • Medical Planning: Develop and implement individualized medical plans of care based on patient needs and clinical findings.
  • Procedures: Perform diagnostic and therapeutic procedures within your level of training and expertise.
  • Medication Management: Prescribe medications as appropriate and manage medication regimens for optimal patient outcomes.
  • Documentation: Ensure timely and accurate documentation of patient care, including billing and medical records.
  • Collaboration: Work collaboratively with supervising physicians and accept feedback and direction to enhance patient care.
Qualifications
  • Education: Certified Nurse Practitioner (NP) or Physician Assistant (PA) with WI state licensure.
  • Valid Wisconsin DEA.
  • Experience: At least 1 year of experience in internal medicine, geriatrics, or similar fields is preferred.
  • Skills: Strong diagnostic and clinical skills, excellent communication abilities, and proficiency in performing medical procedures.
  • Attributes: Compassionate, detail-oriented, and capable of working effectively in a team-oriented environment.
