Optimus Healthcare is a community-driven health center dedicated to providing high-quality, affordable healthcare to underserved populations. We are seeking a Licensed Practical Nurse (LPN) to join our team at our Park City Primary Care Clinic in Bridgeport, CT.

Job Summary

The LPN will work under the direction of the Nurse Manager and within the framework of supportive and restorative care, health counseling, and teaching. The successful candidate will be responsible for performing selected tasks, collaborating with the healthcare team, and executing the medical regimen under the direction of licensed primary care providers.

Responsibilities
  • Perform selected tasks and share responsibility under the direction of the Nurse Manager and within the framework of supportive and restorative care, health counseling, and teaching.
  • Collaborate with the healthcare team to ensure a complete and timely patient flow process.
  • Execute the medical regimen under the direction of licensed primary care providers.
  • Provide health education and counseling to patients and families.
  • Document patient information accurately and efficiently.
Requirements
  • Graduated from an accredited school of Licensed Practical Nursing.
  • 1-3 years' experience as an LPN, preferably in a culturally diverse patient population.
  • Bi-lingual English/Spanish is a plus.
  • Valid and current LPN license to practice in the State of Connecticut.
