Work Schedule: 40 hours per week, 8:00 AM - 4:30 PM

Key Responsibilities

The Case Manager RN is accountable for overseeing the quality and resource management of all patients admitted to the facility, from the point of admission through the entire continuum of healthcare management. This role serves as an advocate for patients, promoting cost-effective care while demonstrating leadership to ensure a seamless integration of healthcare services.

The responsibilities include:

  • Facilitating and ensuring the achievement of quality, clinical, and cost-effective outcomes.
  • Conducting comprehensive admission and concurrent reviews of medical records to assess medical necessity, service intensity, and illness severity.
Qualifications

Education and Experience Requirements:

  • A minimum of 5 years of post-graduate experience from an accredited nursing program and a current state Registered Nurse license.
  • For candidates grandfathered prior to specific regulations, a minimum of 5 years post-graduate experience from an accredited social work program is acceptable, though RN Case Manager is preferred.
  • Preferred experience includes five years in acute care nursing and at least one year in case management, discharge planning, or nursing management.
  • Current BCLS (AHA) certification is preferred.
  • Familiarity with Milliman and InterQual Criteria is advantageous.
  • Basic to intermediate computer skills are required.
  • Completion of Behavioral Violence Prevention Training within three months of hire is necessary.
