Job Title: MDS CoordinatorWe are seeking a highly skilled and experienced MDS Coordinator to join our team at Seven Oaks Nursing & Rehabilitation. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality of care for our residents.
Responsibilities:- Schedule residents for appropriate admission, annual, quarterly, Medicare assessment or change-of-condition assessments, and oversee communication to nursing and other departments.
- Assure compliance with Federal and State regulations regarding completion/coordination of the comprehensive assessment (MDS).
- Monitor MDS and care plan documentation on all residents, ensuring that complete, orderly chronological and current documentation is in file.
- Maintain proficiency in software and coordinate regular Medicaid meetings to discuss current residents, pending residents, and recipients.
- Perform care plan updates as needed and provide the Quality Indicators Report to DON, Administrator, and pertinent staff.
- Transmit the MDS on a timely basis and remain current with changes in the industry through attendance at outside workshops and conferences.
- Perform Ancillary management, collection, and reporting to corporate billing personnel.
- Licensed Nurse in good standing.
- Geriatric nursing background.
- Restorative care philosophy, excellent professional skills (both assessment and hands-on), and good communication and organization skills.
- Working knowledge of nursing home regulations.
- Ability to work independently as well as a meaningful member of the team.
- Excellent organizational skills to coordinate multiple projects simultaneously.
