Veteran Support Services Case Manager

Catholic Charities is seeking a dedicated and compassionate Case Manager to join our team in supporting veteran families in need. As a Case Manager, you will play a vital role in guiding veteran families through the housing stability process, connecting them to essential resources and providing direct assistance as needed.
Key Responsibilities:
- Conduct respectful and thorough intake and evaluation of assistance requests
- Process assistance requests in a timely manner, adhering to Catholic Charities policy and grant requirements
- Maintain accurate and up-to-date client records, in accordance with Catholic Charities policy and grant requirements
- Develop and implement effective Service Plans to support veteran families
- Prepare and submit written reports, as required by Catholic Charities policy and grant requirements
- Provide information and referrals to additional internal and external support resources, as needed
- Maintain knowledge of current community social needs and applicable government regulations
- Represent Catholic Charities on relevant community-based committees or task forces, as assigned
Requirements:
- Bachelor's degree in Social Work or a related human services field
- At least two years of experience in a front-line human services role
- Strong customer service skills and ability to work effectively with diverse populations
- Proficiency in oral and written English language; proficiency in Spanish is a plus
- Ability to learn and utilize grant-specific web-based applications
- Strong attention to detail and ability to work in a fast-paced environment
Working Conditions:
- General office setting, with occasional community-based work
- Ability to drive to other Catholic Charities or Diocese of Allentown locations, as needed
- Must be willing and able to travel to conferences, trainings, and other events, as required
Clearances and Trainings:
- Prior to start date, completion of Mandated Reporter Training (MRT) and periodic renewal, as required
- Prior to start date, completion of Protecting God's Children Training (PGC), as well as any periodic required trainings and/or recertifications
