**Key Highlights:**

* Physician-led, patient-centered team-based environment
* Provide care across the continuum, focusing on value-based care and supported by a robust team of specialists
* Full-time, schedule can be 4 or 4.5 workdays per week
* Open to BC/BE Internal Medicine physicians
* Must be comfortable working with advanced providers

**Required Qualifications:**

* Unrestricted medical licensure in the state of Ohio or able to obtain prior to start
* Board certified or board eligible in Family Medicine
* Active and unrestricted DEA License or ability to obtain prior to start
