Key Responsibilities of the Care Coordination Supervisor:

In this role, you will assist the Manager in overseeing departmental operations while addressing the needs of team members, which includes participation in annual performance evaluations, recruitment, coaching, and the counseling process. You will also be responsible for managing patient concerns effectively.

Ensure compliance with all regulatory standards relevant to the department. Monitor various payors and lengths of stay, identifying opportunities to decrease lengths of stay through effective case management and the identification of safe alternatives to inpatient care.

Support and promote the strategic objectives and goals of BayCare. Collaborate with ancillary departments, community organizations, and administrative teams to enhance efficiency and effectiveness across the care continuum. Adapt to shifting priorities in a dynamic work environment.


Preferred Qualifications:

Possession of certifications such as CCM (Case Manager Certification), ACM (Case Management Certification), CMAC (Case Management Administrator Certification), or CMGT-BC (Nursing Case Management - Board Certified) is highly desirable.

A Master's degree in Social Work, Mental Health Counseling, Community Counseling, Human Services, or a related field is required if not holding an RN License.


Position Details:

Location: St Josephs Hospital - Tampa, FL
Status: Full-time
Schedule: Monday - Friday, 8:00 AM - 4:30 PM
Weekend Requirement: On-call every third weekend

Certifications and Licensures:
Required: RN (Registered Nurse) or a Master's in Social Work, Mental Health Counseling, Community Counseling, Human Services, or a related field in lieu of RN License
Preferred: LCSW (Licensed Clinical Social Worker) or LMHC (Licensed Mental Health Counselor)


Education:

Required: Associate's degree in Nursing or a Master's in Social Work, Mental Health Counseling, Community Counseling, Human Services, or a related field
Preferred: Bachelor's degree in Nursing or Healthcare Management


Experience:

Required: Minimum of 3 years in Acute Care or Case Management/Discharge Planning
Preferred: At least 1 year in a leadership role
