Job Summary

The AleraCare team is seeking a Per Diem Infusion Nurse Practitioner to join our Los Angeles metro area clinics. This is a travel position with a competitive hourly rate, offering flexibility and occasional travel opportunities.

Responsibilities

Provide nursing services to assigned patients in accordance with the patient's plan of care.

Responsible for the clinical record of each patient receiving nursing care, ensuring progress reports are made to the physician for patients receiving nursing services when the patient's condition changes or deviations from the plan of care occur.

Adhere to the facility's policies and procedures, including patient care policies.

Perform initial patient evaluations/assessments and develop a written plan of care, regularly reassessing the patient's condition and nursing needs and updating the plan of care as necessary.

Furnish those services requiring substantial and specialized nursing skills, initiating appropriate preventive nursing procedures and preparing clinical and progress notes, coordinating services, and informing the physician and other personnel of changes in the patient's condition and needs.

Counsel the patient and family in meeting nursing and related needs, demonstrating competency in providing patient care and providing care in accordance with professional standards and the state's nurse practice act.

Initiate case conferences and communicate the patient plan of care with team members, evaluating and re-evaluating the need for services, providing appropriate patient education and teaching materials, and participating in the Performance Improvement program as requested by the Performance Improvement Coordinator.

Requirements

Graduate of an accredited school of nursing and current NP state license, at least one year of nursing experience, infusion nursing experience preferred, effective interpersonal, time management, and organizational skills, strong communication skills in speaking and writing, organizational skills sufficient to maintain consistently accurate records, ability to recognize and direct information to the appropriate health care provider, computer skills that include word processing and efficient use of the internet and e-mail, current CPR card, health certificate, valid driver's license, reliable transportation, automobile insurance.

Work Environment

The employee is regularly required to stand, walk, sit, talk, and hear, and is required to use their hands to operate vehicles and office equipment, and must occasionally lift and move up to 30 pounds, with specific vision abilities required by this job including close vision, distance vision, peripheral vision, and the ability to adjust focus.
