In this role, you'll be responsible for coordinating and managing care for patients across the continuum of care. Responsibilities:
  • Conduct comprehensive assessments of patients' health and psychosocial needs
  • Develop and implement patient-centered plans of care to achieve quality and cost-effective outcomes
  • Collaborate with patients, families, and healthcare providers to ensure effective care coordination
  • Identify opportunities for coordination and efficient care, and advocate on behalf of patients to achieve the best possible outcomes
  • Provide self-management support for high-risk and complex patients and families
Requirements:
  • Bachelor of Science in Nursing
  • 5 years of experience in nursing, with a focus on infusion therapy and care coordination
  • Unencumbered Registered Nursing License for Ohio
  • Demonstrated understanding of family-centered care and ability to work collaboratively
  • Strong commitment to patient experience, service, and quality
