Position SummaryUnder the supervision of the Access Manager, the Registered Nurse acts as a care coordinator for designated patients. This role involves assessing and addressing patient and family needs, providing preventive and supportive care to those facing terminal illnesses. The hospice nurse employs assessment, intervention, and educational skills to support the patient and family unit.
Key Responsibilities- Conducts admissions processes across all Alive Hospice services.
- Performs comprehensive physical, psychosocial, and spiritual evaluations of patients and their families.
- Utilizes the nursing process to develop and implement a hospice care plan in partnership with interdisciplinary team members.
- Proactively identifies potential complications and initiates interventions to alleviate physical and psychological discomfort, ensuring optimal well-being.
- Delivers skilled nursing care to patients.
- Educates patients and caregivers about the care plan, its objectives, and the implications of diagnoses and prognoses.
- Guides patients and caregivers in resource conservation techniques to support home care.
- Acts as a liaison between Alive Hospice and the patient's primary healthcare provider.
- Assesses the need for and organizes home equipment, medications, and supplies in accordance with Alive Hospice policies.
- Reports any unusual or concerning patient or caregiver issues to the Access Manager and other interdisciplinary team members.
- Supervises and mentors the performance of CNT/HHA and other designated staff.
- Attends to patient deaths, following Alive Hospice protocols, and provides support to caregivers.
- As needed, covers after-hours care or staffing emergencies.
- Completes all clinical documentation promptly in line with Alive Hospice policy.
- Participates in interdisciplinary care planning conferences and team meetings as required.
- Collaborates with team members to ensure effective and compassionate support for patients and caregivers.
- Notifies relevant staff of any changes in patient status as per Alive Hospice procedures.
- Functions as a case manager within the patient care team, working with others to achieve desired patient outcomes.
- Demonstrates effective resource management and understands the financial implications related to the agency's budget.
- Communicates with physicians, medical directors, and other healthcare professionals regarding patient assessments and responses to care plans.
- Serves as a clinical mentor for professional students upon request.
- Supports the agency's commitment to education and research in hospice and palliative care.
- Participates in weekend and after-hours admissions as assigned.
- Additional duties may be assigned as necessary.
Oversees CNT/HHA in home care settings, ensuring compliance with organizational policies and applicable laws.
QualificationsEducation and Experience: Current RN license in Tennessee is required; BSN preferred. Candidates should have at least one year of nursing experience or equivalent. Experience in hospice, oncology, home health, or long-term care is advantageous.
Skills: Strong communication and relationship-building skills, with the ability to maintain patient confidentiality. Critical thinking and problem-solving abilities are essential.
Certificates and Licenses: Must hold a valid Tennessee RN License. Hospice certification is preferred. A valid driver's license and automobile insurance are required for driving duties.
Physical and Work EnvironmentThe role involves regular exposure to infectious diseases and requires frequent driving. Employees may encounter various environmental conditions, including moderate noise levels and the need to perform physical tasks such as lifting and moving patients.
Continuing EducationThis position requires completion of 15 hours of continuing education annually, focusing on topics that enhance job performance and personal growth.
