Job Title: Staff Nurse

Abbott House, LLC is a Specialized Mental Health Rehabilitation Facility (SMHRF) located in the heart of downtown Highland Park. We are a therapeutic facility accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF). Our mission is dedicated to providing accessible, safe, efficient, and effective psychiatric rehabilitation services to individuals with chronic and persistent mental illness.

Job Summary:

We are seeking a qualified Staff Nurse to join our team. The successful candidate will be responsible for providing high-quality patient care, adhering to therapeutic standards, and communicating effectively with the interdisciplinary team. The ideal candidate will have a strong commitment to delivering exceptional patient outcomes and a passion for working in a dynamic and supportive environment.

Responsibilities:
  • Assure quality of care by adhering to therapeutic standards and measuring health outcomes against patient care goals and standards.
  • Communicate with the interdisciplinary team any changes in condition of residents that may require a change in plan of care.
  • Communicate with the D.O.N. and/or physician changes in condition of person served to obtain appropriate orders for continued care.
  • Utilize the Communication Log daily.
  • Participate in orientation of new nursing staff employees.
  • RNs may act as Qualified Mental Health Professionals (QMHPs) if the QMHP is absent from the facility; LPNs may act as Mental Health Professionals (MHPs) if the MHP is absent from the facility.
  • Demonstrate computer proficiency and efficiency with the use of electronic records.
  • Complete monthly nursing summaries, nursing goals for Care Plans, quarterly assessments for medications, and ongoing progress notes.
  • STAT labs, including specimen collection, ordering x-rays, EKG, etc.
  • Operate within the nursing scope of practice in line with principles of best practice and clinical governance.
  • Complete Infection Control Reports for any resident prescribed antibiotics.
  • Log the destruction of expired and discharged medication in the log book. Another nurse must witness the destruction of controlled substances, and both nurses must sign the Destruction of Medication Log Book.
  • Complete POS and MAR for half of the residents on the floor on the 14th of each month.
  • Assume the lead role when responding to medical and psychiatric emergencies.
  • Follow protocol for hospitalization of a resident in an emergency.
  • Follow protocol for infection control, needle safety, etc.
  • Schedule appointments and transportation for resident appointments outside of the facility.
  • Log refrigerator temperature in the log book.
Requirements:
  • Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Illinois.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively with the interdisciplinary team.
  • Basic computer skills and proficiency in electronic medical records.
