Role Purpose

The Advanced Home Healthcare Triage Nurse is responsible for providing after-hours care to patients, evaluating their needs, and furnishing hospice and supportive services.

This role involves receiving calls from clients, assessing patient information, and determining if a visit is necessary. The nurse will also document care given and time spent on phone calls, as well as track problems with patients and trends for staff education.
