Responsibilities- Conduct initial assessments of patients and their families to determine home care needs.
- Provide comprehensive physical assessments and histories of current and previous illnesses.
- Regularly re-evaluate patient nursing needs and make necessary revisions to their care plans.
- Develop and implement individualized care plans that address patients' unique needs and goals.
- Administer medications and treatments as prescribed by physicians and provide direct patient care as defined by the State Nurse Practice Act.
- Counsel patients and their families on meeting their nursing and related needs.
- Provide health care instructions to patients as appropriate based on their assessments and care plans.
- Identify discharge planning needs as part of the care plan development and implement prior to discharge.
- Act as a Case Manager when assigned by Clinical Supervisors and coordinate patient care for assigned caseloads.
- Graduate of an accredited school of nursing with a minimum of one year of experience in a clinical setting.
- Current licensure in California and CPR certification.
- One year of home health care experience preferred.
- Competitive pay structure with a range of $100-$170 per visit.
