About the Role

We are seeking a seasoned RN Clinical Manager to oversee patient care services provided by clinical personnel in our Home Health division. As a key member of our team, you will be responsible for coordinating and managing clinical operations, ensuring quality and compliance with healthcare regulations.

In this role, you will develop and implement clinical strategies, manage caregiver assignments, and conduct staff training while participating in performance improvement initiatives. You will serve as a liaison between the care team, patients, and physicians to ensure the delivery of exceptional home health services.

As a leader, you will be accountable for overseeing the delivery of care to all patients served by the location, receiving case referrals, reviewing available patient information, and making decisions on patient admission. You will also instruct and guide clinicians to promote effective performance and delivery of quality home care services.
