Hospice Social Worker

The Hospice of Santa Cruz County seeks a compassionate and dedicated Social Work Associate to join our Interdisciplinary Group. As a key member of our team, you will be responsible for co-case management of hospice patients and their families, developing and coordinating individualized plans of care that reflect patient-centered goals.

- Key Responsibilities:
- Develop and coordinate individualized plans of care reflecting patient-centered goals
- Provide co-case management support for issues related to caregiving, advance directives, funeral planning, community resources, and insurance benefit oversight
- Collaborate with the Interdisciplinary Group regarding changes in patient status and plan of care
- Exercise professional expertise in managing and coordinating care between and among involved parties in home, SNF, and RCFE settings
Requirements:
- Baccalaureate degree in social work (BSW) from an institution accredited by the Council on Social Work Education
- Minimum 1 year of social work experience in a healthcare setting required; previous experience in hospice or other end-of-life care and services preferred
- Solid experience and/or aptitude with computer systems and electronic medical record systems and charting
Estimated Salary: $55,000 - $65,000 per year, depending on experience and qualifications. We offer a competitive benefits package, including health insurance, paid time off, and opportunities for professional growth and development.
