Job Overview

Position Summary:

The PRN Nurse Practitioner plays a vital role in conducting Face-to-Face Encounters in various settings, including patients' residences and healthcare facilities, to assess ongoing eligibility for Medicare hospice benefits. This position involves close collaboration with the Medical Director of Peoples Hospice, although the Nurse Practitioner will not be responsible for certifying terminal illnesses.

This role is classified as PRN.

Key Responsibilities:

  1. Conduct Face-to-Face Encounters for Medicare Hospice Beneficiaries entering their third benefit period and beyond.
  2. Work in conjunction with the RN Case Manager and/or RN Patient Care Coordinator to identify patients for the Face-to-Face Encounter and establish timelines for completion.
  3. Engage with team members on an as-needed basis to gather clinical insights regarding patients and their families.
  4. Review patient records and relevant medical information prior to conducting the Face-to-Face Encounter.
  5. Document clinical findings from the Face-to-Face Encounter using designated forms provided by the hospice.
  6. Ensure that the clinical findings from the Face-to-Face Encounter are communicated to the certifying physician to assist in determining ongoing eligibility for hospice care.
  7. Comply with company policies regarding the protection of health information.
  8. Guarantee the accuracy and promptness of documentation, including billing and clinical notes.
  9. Exemplify the values and mission of Peoples Hospice within both professional and community settings.
  10. Perform additional duties as assigned by the Vice President of Operations and Medical Director.

Qualifications:

  1. Must be a Registered Nurse with a valid license to practice as an Advanced Practice Nurse in the State of Florida, accredited by an approved certifying body for advanced practice nursing.
  2. Possess comprehensive knowledge and well-honed skills in:
  3. Medicine and Pharmacology
  4. Pain and Symptom Management
  5. Psychological aspects of loss
  6. Hold a valid Florida Driver's License and have automobile insurance.
  7. Demonstrate understanding and acceptance of the principles of care provided by Peoples Hospice and Palliative Care.

Work Environment:

In this role, the employee will travel by automobile and may encounter varying weather conditions. The work environment includes diverse residential and healthcare facility settings based on the assigned caseload.

Physical Requirements:

While fulfilling the responsibilities of this position, the employee must be able to see, talk, and hear. Regular tasks include standing, walking, sitting, repositioning patients, using hands and fingers to manipulate objects, and reaching with hands and arms. The employee may frequently lift and move items weighing up to 50 lbs.
