Position Overview

Creekside Health and Rehabilitation is seeking a dedicated Certified Nursing Assistant (CNA) to contribute to our mission of providing exceptional care.

*We are currently looking for full-time evening shift CNAs and welcome PRN CNAs for all shifts.

As a Certified Nursing Assistant (CNA), you will play a vital role in delivering essential nursing support and assisting residents with their daily living needs.

Key Responsibilities

  • Respond to resident calls to assess their needs.
  • Assist residents with personal care tasks including dressing, grooming, and hygiene.
  • Support residents during meal times by providing assistance with eating.
  • Facilitate mobility by assisting with exercises and movement to/from wheelchairs or activity areas.
  • Help residents with positioning and turning in bed.
  • Assist with ambulation as per facility guidelines.
  • Monitor and record vital signs and intake/output as directed.
  • Document daily activities and personal care provided to residents.
  • Participate in interdisciplinary meetings to contribute to care planning.
  • Ensure compliance with documentation standards for restorative services.
  • Collaborate with facility staff to deliver personalized care based on resident preferences.
  • Maintain punctuality and regular attendance for assigned shifts.

Additional Responsibilities

  • Carry out other assigned tasks as needed.
  • Adhere to company policies, federal and state regulations, and professional standards.
  • Demonstrate quick and effective decision-making in emergency situations.
  • Communicate clearly and compassionately with residents, families, and staff.
  • Uphold resident confidentiality at all times.

Qualifications

Education

  • High school diploma or equivalent is required.

Licenses/Certification

  • Must possess a valid CNA certification in the state of employment.
  • CPR certification is preferred.

Experience

  • Preferred: Six months of experience in a long-term care setting.

Note: All employees must complete background checks as per state requirements before engaging with residents.
