Job Title: Behavioral Health Case Manager II

Join Elevance Health as a Behavioral Health Case Manager II and make a meaningful impact on the lives of our members. As a key member of our team, you will be responsible for providing case management services to individuals with complex behavioral health and substance abuse needs.

Key Responsibilities:
  • Conduct comprehensive assessments to identify individual needs and develop personalized care plans.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
  • Collaborate with internal and external resources to coordinate care and support services.
  • Provide education and support to members and their families to promote healthy behaviors and lifestyle changes.
  • Participate in cross-functional teams to drive quality improvement initiatives and enhance member experience.
Requirements:
  • Master's degree in social work, counseling, or a related field, or a degree in nursing with a minimum of 3 years of clinical experience.
  • Current active unrestricted license to practice as a health professional within the scope of licensure in applicable states or territory of the United States.
  • Previous experience in case management and telephonic and/or in-person coaching with members with complex psychiatric/substance abuse and/or medical disorders.
Preferred Skills:
  • Experience in health coaching and motivational interviewing techniques.
