The Healthcare Services Coordinator plays a vital role in managing medical and psychiatric care for adults with developmental disabilities, ensuring they receive the necessary support to achieve their individual goals.
Key Responsibilities:
Core Functions:
- Safeguard the rights, health, safety, and personal aspirations of individuals residing in the facility at all times.
- Organize and schedule all medical and psychiatric appointments for clients, encompassing routine check-ups, follow-up visits, and urgent care situations.
- Verify appointment locations, arrange transportation, and obtain directions to ensure timely arrivals.
- Ensure all essential documentation for successful appointment completion is prepared prior to departure (e.g., consultation forms, medical records, nursing notes).
- Collect completed consultation forms and necessary prescriptions from healthcare providers before leaving appointments.
- Review the medical appointment calendar daily to monitor scheduled appointments.
- Accurately document treatment and communicate updates to relevant team members.
- Maintain an inventory of medications and ensure timely ordering as required.
- Complete all necessary documentation accurately and promptly.
Additional Duties:
- Accompany and transport clients to medical appointments, advocating for their healthcare needs.
- Fulfill essential duties of a Direct Support Professional (DSP) as necessary.
- Ensure the needs of individuals are met before, during, and after appointments, including hygiene, nutrition, and medication administration.
- Assist medical personnel in delivering training to staff.
- Communicate respectfully with all individuals, including clients, supervisors, team members, and community members.
- Complete all required training and maintain compliance with organizational standards.
- Uphold confidentiality regarding any information related to individuals and employees.
Qualifications:
- Must be at least 18 years of age.
- High School Diploma or GED is required.
- Possess a valid driver's license and meet criteria to drive for the organization.
- Ability to adapt to a flexible schedule based on program needs.
- Demonstrated capability to work collaboratively in a team environment, keeping others informed and anticipating needs.
- Strong organizational skills and excellent communication abilities are essential.
- Proficient writing skills with a solid understanding of basic grammar and spelling.
Physical Requirements:
- Ability to sit, walk, and stand for extended periods (minimum of 8 hours).
- Effective verbal and written communication skills are required.
- Proficient in operating standard office equipment such as computers, phones, and photocopiers.
- Ability to reach with hands and arms, and perform bending, stooping, kneeling, or crouching as necessary.
- Must be able to lift up to 50 pounds.
Compensation:
USD $20.00 - $21.98 per hour
