Position Overview:

The Advanced Practice Clinician (APC) in Neurology will concentrate on delivering extensive post-discharge care primarily through virtual consultations, with a commitment of 8 days per month for in-person clinical responsibilities. This role will prioritize the management of various neurological disorders, particularly focusing on stroke care and other neurovascular issues.

Assessment & Management
  • Conduct comprehensive history and physical examinations, emphasizing neurological evaluations, and accurately document findings in the medical records.
  • Order and interpret diagnostic tests pertinent to neurological conditions, including neuroimaging, EEGs, and laboratory analyses.
  • Recognize and manage risks associated with the treatment of stroke and other neurologically intricate patients, including secondary stroke prevention, post-stroke spasticity, and seizure management.
  • Collaborate with multidisciplinary teams to triage and address acute neurological symptoms, ensuring prompt intervention and follow-up care.
Diagnosis
  • Work in close partnership with the interdisciplinary healthcare team to diagnose and manage neurological conditions, focusing on stroke, TIA, and other cerebrovascular disorders.
  • Oversee and interpret diagnostic tests, developing differential diagnoses for both acute and chronic neurological conditions.
  • Diagnose and manage complications related to stroke and other neurological disorders, such as recurrent stroke, hemorrhagic transformation, and neurogenic dysphagia.
Formulates Plan of Care
  • Create personalized care plans for patients with stroke and other neurological conditions, integrating evidence-based practice guidelines for neurorehabilitation, secondary prevention, and chronic disease management.
  • Implement and modify care plans to meet the evolving needs of patients throughout the continuum of care, from acute recovery to long-term management.
  • Prescribe suitable diagnostic strategies and therapeutic interventions, including medications, physical therapy, and neurorehabilitation services.
  • Perform advanced neurological procedures in accordance with privileges and validated competencies.
Documentation
  • Record essential components of patient care, including detailed neurological assessments, diagnostic results, and individualized care plans. Ensure documentation is timely, meets ambulatory care compliance standards, and reflects the complexity and acuity of neurological patients.
Communication and Collaboration
  • Maintain continuous communication and collaboration with the interdisciplinary neurology care team, including stroke coordinators, rehabilitation specialists, and case managers.
  • Facilitate clear communication with patients, families, and healthcare staff to ensure continuity of care across the inpatient and outpatient continuum, with a focus on stroke and other neurological conditions.
Professional Practice
  • Exhibit professional practice behaviors, including mentoring and educating graduate students, particularly in stroke and neurological care.
  • Engage in research opportunities and contribute to the analysis and application of evidence-based practices in stroke care and neurorehabilitation.
  • Actively participate in Advanced Practice Grand Rounds and neurology-specific meetings, remaining current with continuing education and professional development requirements.
  • Hold a master’s degree in nursing or a related field, with certification as an Advanced Practice Clinician.
  • Possess experience in neurology, with a strong emphasis on stroke care, neurorehabilitation, and post-acute neurological management.
  • Demonstrate a commitment to interdisciplinary collaboration and ongoing professional development in the field of neurology and stroke care.
  • Hold National Board Certification as a Nurse Practitioner or Physician Assistant.
  • Maintain BLS and ACLS certification.
